An in person or online assessment, conducted by an EdHealth diabetes educator, delivers customised solutions for better diabetes care for people with a disability. Supported by a support worker and/or family member, information is gathered about their diabetes, their disability impact on their capacity to self-care and the diabetes support role/s of their circle of support.

We find out about their type of diabetes, lifestyle and medication treatments, glucose monitoring and other health/disability and support issues. Details are noted about the health professionals they see and the diabetes treatment, management and advice they are receiving. A deep understanding of their current diabetes management is gained and gaps in their support and healthcare delivery identified.

A report documents the assessment and outlines strategies to address or improve deficits identified, with the NDIS participant and their circle of support actively involved in strategies and service choice to improve their diabetes care.
